Understanding Abnormal Blood Gas Results: Causes & Next Steps

On my recent labs it indicated, BUN 19 mg/dl (7-18), BUN/Creat ratio 26 (6-20), Anion gap -2.0 (5.0-19.0) CO2 36 mmol/l (21-32),a year ago it was 32mmol/l. Had spiromety which was normal and then was sent for blood gas with the following results:
PO2 110 mmHg (75-100)
HCO3 26.6 mmol/l (22.0 - 26.0)
TCO2 27.8 mmol/l(21.0-25.0)
O2 Sat 98.1%(95.98.0)

I'm going for a consultation with a nephrologist to evaluate my kidneys. What could be possibly going on? Could I have 2 things going on? I'm a 52 year old healthy (until these labs) female, 5'3" 115 lb who exercises daily for 1/2 hour with aerobic and muscle toning. I do have a history of UTI's and was on a pediatric dose of Nitrofurantoin for 2.5 years and wonder if that affected my kidneys in some way.
Besides a nephrologist is there any other medical specialty I should consider? Thanks!

Hi Ann, I'd be glad to try and answer your question.  I think your blood glas results look OK.  Your P02 (oxygen) level is higher than normal, but that's good in this case.  I'd be more concerned with a low P02.  Your O2 sat is very good, which means your body (specifically your blood) has the correct amount of oxygen circulating.  Your BUN and BUN/creatinine ratio are a little high, and those tests could indicate some sort of kidney problem, which is why your doctor is doing the right thing in sending you to a nephrologist.  There may be nothing wrong, but I think you'll feel better going to see the specialist.  I don't see a reason, bsed on the test results you've given me, that you need to consider any other specialist.
