Postmenopausal Bleeding After Thyroid Surgery & HRT: What to Know

Hi, 14 years ago (at age 34) I was diagnosed with a combination of Hashimoto's Thyroiditis and SubAcute Thyroiditis.  My entire thyroid and half of the parathyroid were removed.  I've been taking synthroid since (14 years) and my annual tests show normal levels.

At age 35, I started experiencing symptoms of menopause.  The progestin challenge and high FSH levels confirmed.  I've been on HRT for 14 years.  At each annual exam, my Dr says my uterus is as expected for a post menopausal women.  For the past 5 years, my HRT has been Activella--which is not cyclic.  I never miss a dose. I do still have only minor, manageable menopausal symptoms (night sweats and dry vagina). I haven't had any spotting whatsoever.

Out of the blue about a week ago, I started with mild cramps and spotting of old, dark, clotted blood.  This has been consistent for 7 days.

My question is: is it likely that my system is trying to come back to life?  Or is it more likely a problem that will need medical treatment?  My Dr. says to give it 2 more weeks to see if it gets worse or better.

Hi Jola,
I apologize for the tardiness of my response. I've been out of town for three days. It's OK to wait, but as a rule post menopausal bleeding is a sign that something isn't right. You probably should have a transvaginal/pelvic ultrasound and maybe even a endometrial biopsy just to be sure that nothing is starting up. HRT carries risk of uterine cancer with it, so in my view, an evaluation is mandatory.
