Question -
I am 37 years old, I have 2 children. My last period was September 6, 2004. On october 11, I had a psitive pregnancy test with a progesterone level of 143. The doctor felt this was low, and has put me on prometrium. I began having spotting and became scared, so another test was done, and my progesterone level was 468 on October 18. Now, on October 21, I went in again, still having spotting. They did a sonogram and could not see the baby, and have done another progesterone level, and I do not have my results back yet. When I ask how possible is it that everything will be ok, I get very vague answers. Should I feel this fearful, or are chances good that everything will be ok?
Answer -
If your last period started on September 6, you are now 6 weeks 4 days pregannt. A sonogram at this time should show a gestational sac, a fetus, and a fetal heartbeat. If the sonogram does not show a baby at this time, either your dates are incorrect and you are less than 6 weeks gestation or it is an abnormal pregnancy and you will have a misczrriage. A sonogram taken at less than 6 weeks only shows an empty gestational sac and it may just be too early to see the baby, yet. I would repeat the sonogram in one week to see whether there has been growth.  If your progesterone level was low, Prometrium is a good course to take. YOur progesterone level appears fine now.

There is nothing wrong with an elevated progesterone level. We worry when the progesterone is too low. If your last period started on September 6, you should now be almost 7 weeks pregnant (from the first day of your period). A sonogram at this time should show a fetus and a fetal heartbeat. I would recommend taking another sonogram in one week. If there is no fetus seen, then it is an abnormal pregnancy. HOwever, a sonogram is the best way to follow this pregnahcy. Are you sure that they are measuring progesterone levels and not HCG levels (human chorionic gonadotropic hormone)?  HCG levels should rise every day. Progesterone levels are usually from 10-25. I have never heard of a progesterone level of over 30. Make sure you are talking about progesterone and not HCG.

Answer
After a miscarriage and after you get a normal cycle, you can start trying to conceive right away. When you get a normal period, ovulation usually occurs two weeks after the start of the period and you can start trying to conceive at that time. After a miscarriage, you are not more or less fertile.
