Unexplained HCG Fluctuations in Early Pregnancy: What Could It Mean?

Hi.  I should be 5.4 weeks pregnant.  My numbers have been doubling perfectly.  However when I went for a transvanginal ultrasound at 5.2 weeks, they found nothing.  Nothing in my tubes or uterus.  They said I must have miscarried but already passed it. This seemed to be confirmed by the fact that my HGC dropped in half. Then at a follow up appointment at 5.4 weeks (2 days later), still nothing in my uterus or tubes but my HCG doubled.  What is going on?  They want me to take a metho shot but I am scared.  Why did the numbers drop in half and then double and why can't they see anything anywhere at 5.4 weeks.

Dear Jennifer,

It is very unusual that your hCG would have dropped so dramatically and then rebounded back again. I would guess that they will want to do yet another hCG level to confirm whether it is continuing to rise or whether it is dropping again.

At 5 weeks, we would expect to see a gestational sac and quite likely a yolk sac as well. If you are not sure of your dating by a week or more, this can make a difference in not being able to see anything.

If your hCG continues to rise, I would suspect they will want to wait another week or so and try another ultrasound in case dates were off. If your hCG drops, then it unfortunately sounds more like a probable miscarriage situation.
