Post-IUI Pregnancy Concerns: Cramps & Progesterone - Expert Advice

Hi Brenda

My last period started from 3/5/2007 and I had HSG on 3/8/07 and IUI on 3/17/07 and with vaginal progestrone gel from 3/19/07 upto now. I am 37 years old.

HCG on 3/31/07 = 372.6
HCG on 4/6/07 = 8221 and progestrone is 66

Today, I still have on and off cramps, most of the time is mild cramps like period cramps. Do you think that it is a sign of miscarriages? and do you think that I can stop using the vaginal progestrone gel? Some said I will have the twin based on the HCG figures, what is your opinion?

This is my first pregnancy and I have no experiecne at all. So do you think that I am in good or bad condition, please advise. And during this time, what things I should pay special attention.

The rate of multiple gestation pregnancies with an IUI is approximately 23-30%.

I would estimate that you are approximately 5 weeks, 2 days today. So on 3/31, you would have been 4 weeks and on 4/6 you would have been almost 5 weeks. Normal hCG levels for 4 weeks LMP are 5 - 426 mIU/ml and for 5 weeks LMP are 18 - 7,340 mIU/ml. So, you are definitely riding on the high side which would tend to put up the "twins" flag.

Your progesterone level is also high for your stage of pregnancy, but like the hCG, it would be expected to be higher with a multiple pregnancy. Your doctor is the one to ask about discontinuing use. It may be that you can stop after the 1st trimester, but at this point he/she is likely ordering it to help maintain the pregnancy. Your doctor will want to be assured that the pregnancy is not in jeopardy before it is discontinued.

More than likely an ultrasound will be ordered at approximately 6-7 weeks. You will have confirmation of how many babies you are carrying at that point.  :-)
