Understanding Tremors: A Case Study for Homeopathic Physicians

" Dear Sir,
I am homoeopathic physician. Yesterday I saw the male patient, age 50 years suffering from following complaints.
1. Tremors to both hands, more on right hand since last 5-6 months. Tremors aggravate during fine movements.
2. Inability to perform fine movement, like fastens the buttons of shirts etc.
3. Emaciation of right upper limb. (muscle wasting of right upper limb)
4. Weakness and inability to carry weight by right hand.
5. Intermittent spasms of right hand muscles and calf muscles.
6. Fingers of both hands remained in slight contracted (flexion) position and inability to extend them.

Sir, what would be the probable diagnosis? Which investigations should I suggest to him to confirm the diagnosis?
I suspect Parkinsonism .But there in muscle wasting and intermittent spasms of Right upper extremity , and I think these are not the features of Parkinsonism
So please guide me . Thank You.
