Lower Abdominal Pain & Blood in Urine: Possible Causes & What to Do

QUESTION: I am a 27 year old male.  Today I was awaken with moderate to severe pain across my lower abdomen.  I have had this happen to me about 6 times in my life but most previous episode was about 14 months ago.  The pain lasts about 24 hours then seems to migrate farther down on my right side and fade away after another 24 hours.  On about half of these episodes I have had blood in my urine.  All symptoms will fade and reoccurs at a later date.  With it being 14 months I really thought I was over it whatever it might have been.  Does this sound like Crohns disease or something in the UT?  Again, other than occasional blood and pain there are no other symptoms.

ANSWER: It could be Crohn's... there are a lot of possibilities and the only way to tell is to start the evaluation... the blood in the urine could go along w/ crohn's but also kidney disease, etc.

QUESTION: I am on day 2 of this issue and as predicted the pain settled in the right side about 4 inches above my penis level. I am so sore down there as like I did an extreme ab workout. Sitting up and coughing really make it hurt and when I push to pee it hurts too. Does this happens with crohns? If this follows the trend of all other episodes everything will be gone by tomorrow and wouldn't repeat for at least 2 months.

ANSWER: There are other possibilities too... chronic appendicitis... an appendicitis that flares up then subsides.  That would also contribute to blood in the urine.  But I would not ignore this.... you need to get an answer at some point because one day it is going to get a lot worse and become a medical emergency

QUESTION: I have seen a ton of doctors and no one seems to know. I had xrays, ct scans, and blood work. The only thing that came back abnormal was a high white blood cells in a blood test. When that came back high it was the only time I was able to see a doctor during the pain. Had blood drawn 3 days later when pain was gone and everything was back to normal. They took blood yesterday and had a physical exam and they are still clue less although the blood work is not back. They want me to do another ct scan o my abdomen and ultra sound on my kidneys but I feel like it's pointless cause there will be no symptoms when the test occurs. I am trying my best to get diagnosed but no one knows and I am getting very frustrated

I see.... what it may take is getting the cat scan with contrast while you are actively sick to try an isolate the problem, or getting a colonoscopy or small bowel GI series when you aren't sick to rule out crohn's or ulcerative colitis.

I don't know if you have had an ESR or sed rate.... if that is high, it is non-specific- won't point to a problem, but it tells you a significant problem is there.
