Right-Side Abdominal Pain & Cramping - Postpartum Concern

Question
I am 25 years old. the date of the first day of last menstrual period was 02/25.  i have twin boys who were born at 25 weeks.  i am receiving prenatal care at a high-rish clinic but am having some concerns.  Since last night i have been having sharp pains on the right side right at where my pubic hair starts.  this is not the first time i have experienced cramping, but the first time i have experienced it in this particular area.  also, i have extreme pain in my lower back, right above my butt bone.  I also have a left prosthesis that goes all the way up.  Please let me know what you think it could be.  I have a doctor's appt on Wednesday but i need to know if this is something you think i should go to the clinic sooner.  thank you for time and expertise.
sincerely,
Lacy Atzin

Answer
At 14 weeks gestation, a pain in the pubic area (groin area) is usually due to stretching of the round ligaments. These are the ligaments that anchor the uterus to the sidewall of the abdomen. The pain is worsened upon rising from a bed or chair and if you suddenly turn while walking. It is called the "Round Ligament Syndrome" and will disappear at around 25 weeks. You can alleviate the pain with Tylenol or with a heating pad or a warm shower. The lower back pain is not from the same cause, but is due to the fact that as the uterus is growing forward and your abdomen is growing outward, your center of gravity is moving backward and there is a lot of pressure on your spine, causing low back pain. Again, analgesics will help as will heat. Good Luck with your current pregnancy. Try to drink plenty of fluids and be aware of any regular pains, so that you can avoid a recurrence of preterm labor. I'm sure you will do fine.
