Persistent Post-Miscarriage Bleeding & HCG - Seeking Medical Clarity


Question
As of May 24 bleeding has stopped. Usually stops for 4-7 days, starts up again heavy like period with cramps (HCG is 12)

What does this mean: "May have a subinvoluted uterus and must contract the uterus with Methergine"

What else could be the reason why my HCG levels taking so long to drop?

OB said I may have a cancer transformation.  What's that

On cale of 1-10 how concerned should I be with my health.-------------------------

At 13weeks preganant (Jan 30/06) I had an ultrasound which revealed my baby had dided.  Two days later on Feb. 2/06 I had a D&C done.  I've been bleeding off and on since then at times very heavy with cramping and I also pass quarter size clots.  As of May 30/06 my HCG level was 12.  Two and a half weeks prior to that it was 15.  Two and a half weeks prior to that it was 25.  My OBGYN says that this is uncommon but sometimes occurs and I just have to wait it out.  Last month my family GP sent me for an untrasound and it showed "retained product likely due to conception".  My OBGYN refused to look at the results stating that ultrasounds are inaccurate and she only uses bloodtest results to make sure everything is returning to normal.  However 4 months later and I'm still bleeding is not normal.  I don't have any symptoms of infection.  I'm really concerned about why this is still going on and the possibilty of infertility.  Is this normal?  What if anything can I do?  I'm currently on a waiting list to see another OBGYN for a second opinion.

Answer -
It is definitely NOT common to bleed four months after a D&C for a miscarriage. You may have some retained products of conception, you may need another D&C, you may have a subinvoluted uterus and must contract the uterus with Methergine, etc. If the sonogram showed retained products of conception, that is accurate and you need another D&C. The sonogram is accurate and, if it says there are products remaining in the uterus, it is accurage.

Answer
Since the sonogram showed some retained products of conception, you need to have another D&C  or the uterus will not contract and get back to normal. If you did not have retained products of conception, it may merely be subivoluted (meaning that the uterus has  not contracted down yet). This does not allow the uterus to return to normal. We usually give Methergine 0.2mg everu four hours for a dau and the uterus returns to normal.  During the time you are taking the Methergine, you will have cramps (like you did after the baby was born). It is not Cancer. Don't worry. First try the Methergine, then the D&C.
