Answer
I dont know much about area's outside of Canada. However here in Canada, once you request to have a tubiligation done, you must sign forms to give full consent for the doctor to perform this surgery.

When you had your son, did you have a c-section? usually after c-section births they will often tie them while they have already open, rather them come back a year later, just to have another procedure done.

Now, if you agree'd with your doctor to have your tubes tied, and had a c-section birth with your son, I would give him a call. He would definatly have all procedures done to you on record. And thats the only way you can find out for sure, is to speak to your OB that delivered your son.

If you are unable to do that, ultra sounds would also allow to see what is going on there.

But truthfully, Dr's sit down and discuss the pros and cons of tubiligation before doing the procedure. Its highly unlikely that a dr will ask a patient if they would like to have a tubiligation done, and just perform it without discussion on the procedure, and or the signing of waivers.

Again, I would call your OB and ask him.

Hope this is of some help.
