If I had an anatomical abnormality with my uterus, wouldn't it have showed up by now, considering I have had many transvaginal ultrasounds done? Or can a diagnosis of this nature only be made via an HSG? I have no problems conceiving but have had 2 early pregnancy losses.

Sometimes even an HSG is not enough and a hysteroscope, an endoscope with a camera that shows the inside of your uterus with a light source, is needed to reveal an abnormality inside your uterus. Ultrasound, whether abdominal or vaginal, is not that specific in showing some abnormalities such as septate and bicornuate uterus.
Many of these abnormalities are correctable depending on the type of the abnormality. However, it is important, if your main complaint is recurrent pregnancy loss, to consult a reproductive endocrinologist. Simple causes such as luteal phase defect may be responsible in which some natural progesterone would help.
