Understanding CIN III: Colposcopy, Biopsy & Next Steps

Thank you so much for your fast reply it seems a rare thing these days.I did have a biopsy and colposcopy but am not aware of an ECC.The biopsy was clear and the colposcopy only showed a low grade leision but had a CIN III pap twice leading them to believe the problem was higher in the cervix,which they obviously found during lletz.I am due for another smear and colposcopy in March.Unaware of an ECC.It was meant to be in Jan but the hospital here in Australia that I am attending are busy and wanted me to re book although I was not happy about waiting 6 months intead of the initial 3-4 months they told me.Does the report mean that the CIN III was spreading into the glands ? you say you would advise an ECC because of the endocervical gland involvement.Why ? and what information would this provide ?
Hope you don't mind me asking it's just no one else I have contacted has been very helpfull or even answered and I appreciate it very much.

I have had a LLETZ procedure for CIN III.The report says it was all removed.IT says there is a focus of severe dysplasia CIN III at the transition zone which involves the endocervical gland.I understand the CIN III part but I do not understand the endocervical gland part.I have been hunting the internet and I am unsure if it has anything to do with Adenocarcinoma or AIS.I would really like to understand this.I have a follow up check in March 07 (6 months after lletz ) Any help would be great.

Hi Marie,

High grade dysplasia like you have can involve the endocervical gland and have nothing to do with AIS. Common treatment for AIS is conizations, not LLETZ, so from my understanding you suffered from common high grade dysplasia.

The best thing you can do for yourself is to be sure to make ALL follow up appointments. Dysplasia can return, so it is so important to maintain follow up.

I assume you will be getting another colposcopy/biopsy during your follow up. You may want to ask your doctor beforehand if he or she will be doing an ECC (endo cervical curettage). I am sure they did an ECC prior as it is standard, but you may want to make sure if they are going to repeat. Because of the endocervical gland involvement, I would certainly want it done.

Hi Marie,

The endocervical canal is the space between the cervix and uterus. An ECC will determine if there is any dysplasia higher up in the cervix.

The ECC is standard in most doctor's offices here in the U.S. I would see if this is an option for you, and if not, ask your doctor why he or she does not recommend. It is a very simple procedure, takes less than minutes, and is routinely done with colposcopy and biopsy.
