Understanding V-Tach & Chest Pain: What You Need to Know

im a 25yr old male, and had echo's, stress test, stress echo, holter monitor and event monitor, and everything was PERFECT besides on the event monitor i had PVC's, and a 25 beat run at 127 bpm, of Ventricular Tachycardia. Dr said he thinks it is Idiopathic ventricular tachycardia, he wants to do an EP study to be safe and possibly a ablation. Does it mean since i had that run that my heart is more prone to ventricular fibrillation and SCD? Do you think it is possible to have this and it be benign?

could it be Arrhythmogenic Right Ventricular Dysplasia/Cardiomyopathy? how would that be diagnosed? would that fit my symptoms of chest pain and the PVC's and VT and other symptoms that I feel? im REALLY concerned about could it be Arrhythmogenic Right Ventricular Dysplasia/Cardiomyopathy!!

Just how benign is "Idiopathic Ventricular Tachycardia"? Can it develop into V-Fib? do you know if its possible for me to have right ventricular dysplasia that causes right outflow ventricular tachycardia? would the test that ive had, have caught any of these diseases? im just so scared of SCD and want to know whats going on. i have the palpitation feelings all the time and a funny feeling in my chest over my heart, and a funny feeling in my throat from these palpitations. I also been having angina like pain, chest, left arm for like 2 yrs but a normal work-up with the above test? does this mean anything since the V-tach was found? is that long of a v-tach run lethal or can become lethal?? me and my fiance are about to have a baby any day now and i want to be there for them. please help

It seems like since this all has started, the left side chest pain and the palpitations, PVC's etc., it has just been getting worse over time, I know that ARVD is a progressive disease and this is the only thing that I'm SUPER concerned about, I would greatly appreciate it if i  could fax the test results to you and let me know if there is anything that could suggest ARVD, such at eplison waves, inverted waves in the wrong spot on an ekg or event monitor.  Please give me some info on this subject.

I just recently went to Houston, TX about 3 weeks ago and seen another Electrophysiologist, and he had me do a Cardiac MRI with contrast, and all my results on that was perfect.  He said no heart disease or ARVD was detected or suspected at all from the cardiac MRI. I asked about the EP study and he was kinda like "if you want to we can" and I said well do you suggest it and he said not really.  He acted like the v-tach that I had captured on the event monitor wasnt of any concern, and he said if you put a monitor on the entire world that 20% of the population would have some v-tach.  I just can't understand this, and I feel that it will lead to v-fib.  Can it? Can excessive alcohol lead to v-fib in someone who has had documented v-tach already?  Can electrolyte imbalances lead to v-fib in someone like me or is it IMPOSSIBLE for someone with a "NORMAL" heart?

You got it right in the first paragraph.... you might be more prone to sudden cardiac death.  You need to be evaluated for Brugada's Syndrome and others via that EP study.

Idiopathic means "we don't know the cause."  VT of any type can erode into FV.  RV disease may be the cause.  This gets into highly specialized medicine of which I am not an expert, but an EP study will map out the nerve networks in the heart that are diseased and it is very treatable/fixable.  

People have PVC's and short VT runs all the time.... it is very common.  The longer ones are the worrisome ones... you could theoretically be in VT for hours or days but the nature of VT means it can slowly or quickly decay into VF/SCD.

I don't know much about RVD and can't advise.

The electophysiologic cardiologists don't seem to get that excited about VT runs for some reason.  PVC's occur in everyone but long runs don't.  Yes, electrolyte imbalances definitely can cause them or make them worse, so your potassium needs to be normal and I would suspect they have you on Magnesium 400 mg twice a day.  

Get the EP study asap.
