VBAC Access & Concerns: Navigating Obstetric Care After a C-Section

Question
I live in a major US city that has recently had a mass exodus of OBs--cannot find an OB who will perform a VBAC (or for that matter, a hospital who will do so willingly--most do not have the surgical team/anesth. available, with the exception of an inner city teaching hospital).  Doctors here make it sound as if VBAC is one of the dumbest things you can do if you are having a baby.  I am 35 years old with one prior c-section 7/03 at 38 wks (7lb 6oz) with oligohydraminos, breech presentation and hx of GD, BV and anemia.  Based on previous hx and age, I dont know that I am comfortable with a midwife/homebirth. I had such a long recovery and a lot of pain from my last c-section and dont to have another.Do you have any suggestions?
Thanks
Jodi

Answer
If your cesarean section wax performed due to breech presentation, there is no reason why you cannot try for a VBAC. However, the trend in many major cities in the U.S. is to discourage VBACs recently, primarily due to lack of immediate surgical teams, anesthesiology, etc (as you mentioned). However, with diligence, you should be able to find an obstetrician that will allow you to attempt a VBAC. Certainly, your risks are minimal. I'm sure that you are aware that the risk of uterine rupture along the previous scar is 1%. Other than relocating to an area where VBACs are the norm, I have no other suggestions. Good Luck.
