Can you get pregnant while on birth control and using a condom and spermicide?

My friend said that she was on Birth Control and she used a condom and spermicide while having sex and she got pregnant.

What are the chances of that happening?

Since birth control pills are 99.9% effective against pregnancy, there is less then 0.1% chance of becoming pregnant while on birth control pills. If, in addition you are using condoms and spermicide, there is less chance of becoming pregnant. However, there is always a possibility that any method will fail to prevent pregnancy.
