Irregular Periods & Fertility Concerns: Seeking Information

Question
I am a 40 year old childless black female. About six months ago my menses, which normally came every month, began to come every other month and once was absent for two months. My last menses began 10/03/05. I have a normal menses blood flow, duration, and no pain. I desire to have two children while I hopefully still can.
I have two large fibroids that cause frequent urination, abdominal pressure, and no physical pain. One is inside the uterus measuring 6.7cm x 7.9cm x 8.7cm, and the other is pedunculated with a thin stalk setting on top the uterus measuring 10.6cm x 14.4cm x 14.8cm. About five years ago I had a miscarriage in my first month of pregnancy that my OBGYN said was directly due to the fibriod inside the uterus. I have no other known pregnancies or miscarriages.

Beside my fibroidal uterus, I am a very healthy 40 year old woman in that I do not smoke or drink or have any allergies, and no other known health problems.

I have been dealing with my fibroid problem for a very long time. Thus, I am aware of and researched and considered all the surgical and non-surgical options for removal.

I frequently search the internet for any up-to-date info on fibroid treatment. Recently, on the internet, I came across a medication called Neprinol that has shown success in shrinking  both large and small fibroids.

I would like to avoid a myomectomy, but will have one as a last recourse.

Neprinol sounds like a wonderful treatment, mainly in that it's non-surgically invasive.

What is your advice on Neprinol to shrink fibroids?

If Neprinol is a good treatment, is it successful on pedunculated fibroids?

My main concern is my thin stalked pedunculated fibroid because I understand that if it detaches from the uterus and becomes free-flowing it will likely later cause problems that will require surgery.

Any and all advice you can give me will be greatly appreciated.

Thank you,
Alfredia

I have an appointment with my OBGYN on 10/09/05, and will discuss Neprinol with her.

Answer
I have never heard of Naprinol nor is it mentioned in the PDR (Physicians Desk Reference), so I can't comment on its use. If y8u find out its generic name or what it contains, I can discuss it with you. Usually, pedunculated fibroids do not cause any proboems with a pregnancy. They do not shear off and travel in the abdominal cavity. Usually the stalk gets thicker and thicker and it remains attached to the outside of the uterus. The fubriud that you have inside the uterus, may be the cause of the difficulty in maintaining a pregnancy if it interferes with the growth of the fetus or impedes its movement towards the cervix. You should consider myomectomy. HOwever, even after myomectomy, the fibroid can grow back, so it is usually recommended to have the myomectomy, become pregnant, and have your baby before the fibroid regrows. Discuss it with your doctor.
