Question
I am 21 years old and have a 17 month old girl. History- 3wks after start trying to conceive was admitted to hosp with lower left abdo pain, Suspect ectopic. Confirmed HCG positive 3 mIU/ml. After scan - Pain diagnosis ovarian cysts. Suffered whole pregnancy. Normal delivery, healthy baby. Started trying for another baby 3mnths after daughter born. Breast-fed 10mnths no period.  Period returned after discontinued b-feed, very heavy, irregular for 3month.
Now -regular- 28 day cycle. 1-2wks before period due I suffer lower left and right abdominal pain. Sore, stabbing, deep pain comes and goes. Period always very heavy 3days, very bad menstrual pain.
After intercourse suffer watery diaharrea. Deep penetration causes pressure-like discomfort inside, sometimes severe pain. Also suffer random stabbing pains in my lower left back parallel lower left front pain, but come at different times.
Been to doctors numerous times.
1 sexual partner, No infections, most recent scan no cysts, blood fine. Gyn diag IBS. Not convinced. Still not pregnant.
My period was 4 days early this month, very painful heavy flow. finished 10 days ago, yesterday pink, mucus discharge, today dark brown discharge. No odour. No itching, burning. Same abdo pain. Any idea? Do you think endometriosis possible and how can I get it investigated? I was told my symptons are not related. Thank you. Sarah.

Answer
If a sonogram showed no evidence of ovarian cysts, and you have gastrointestinal symptoms from time to time, you may, indeed, have IBS. I doubt that you have endometriosis, which would only cause pain during your period. Most of the time, the pain is low down just above your pubic bone. However, it can be located on one side of the pelis or the other. The only way to definitively diagnose endoemtriosis is via laparoscopy where we actually can see the endometrial implants ("powder bunr residue"). All of your other symptoms appear within normal limits, including the discharge.
