Tretinoin Use During Pregnancy: Risks & Safety

My dermatoligst said it was safe to use Tretinoin gel while I was pregnant.  However I read else where that it wasn't safe.  So who is right?

Dear Laura,

According to the Organization of Teratology Services, which examines the effects of different medications and other factors on fetal health, there is a slightly higher than normal risk for a birth defect with tretinoin (Retin-A) topical medication. With any pregnancy, there is a 3-5% chance of a birth defect. In the OTIS information, there were 4 reports of babies born with birth defects to mom's who used Retin-A, so this tends to fall within the average risk.

This drug belongs to a family of medications called the retinoids. Part of the retinoid family is Accutane. Accutane, taken by mouth, has been known to carry a high risk of birth defects so it is not recommended in pregnancy. However, Retin-A is topical and typically less than 10% of Retin-A is absorbed into your bloodstream. This is what causes the difference. Your dermatologist presumably has the most up-to-date information on this medication and feels comfortable in continuing to prescribe this medication for you. In researching this, I would not necessarily disagree with his decision.
