Early Pregnancy Ultrasound Interpretation & HCG Levels: A Case Study

QUESTION: thanks for your time,
i need your opinion,
i will give you my information and hopefully you can help me.

lmp was may 18 (but not regular bc i got off bc pills and it hasnt been regular)

two days of intercourse.. june 4th(did not pull out) and june 12(pulled out)

information:
found out on june 23(just 12 days after the 12th date) i was pg had hcg betas done that same day it was 923.. on june 24 it was (1530)

july 6 2010 vaginal ulrasound done. measured 6 weeks 2 days,due date was 2-27-11

july 30 2010 vaginal ultrasound done measured 10 weeks 0 days due date was 2-25-11.( I FIGURED THIS FALLS ON JUNE 4TH FOR CONCEPTION)

had a gender scan done on my own (not at docts office) sept 4th 2010, measured 15 weeks 1 days. its a boy!

had my 20 weeks scan by doct. measured three measurments head , ac, femur. still stayed around 2-25, 2-27 for due dates. femur was 2 days behind.

i made a bad mistake on the 12th. do you think i got pregnant on that date..

ANSWER: Based on this information you gave me it does look like you conceived from your sexual encounter on June 4th.  The first trimester gives a range from the measurent of +/-  5 days.  In my opinion, it seems more likely that you got pregnant from June 4th, but if there is a question of paternity, a paternity test should be done to be 100% sure.

one more question,
at my first ultrasound i would have been just 5 weeks 2 days if i got pg from the june 12th encounter, do you think i would have seen a 0.45 cm fetal pole with a heart beat of 132. that early? i measured 6 weeks 2 days that day.doct said conception was june 5th)from that u.sound also i read its only off by 3 days is that true?

To answer your question, no you would not see a fetal pole with detectable heart rate at 5 weeks and 2 days, making that ultrasound you had at 6 weeks and 2 days very accurate for dating purposes.   But if you would have waited until the 10 week ultrasound for the first one, there would be more of an allowable difference of 3 to 5 days.  Still accurate enough for dating but you realize how even more accurate the 6 week ultrasound is.

It depends who you ask, or in my case who you work for of the 3 day difference versus 3 to 5 days.  I would like to think a first trimester ultrasound is perfect to the very day with no difference at all!  But I know that probably doesn't happen all of the time, which is why it is an estimation and should never be assumed as exact.
