I am 34 yol, nullipara, diagnosed with possible PCOS based on multiple peripheral follicles on USG. However formal blood workup is pending. Tried clomid 50 mg and IUI cycle 1 and had a chemical pregnancy, HCG 5 and progesterone 5 on d14 after IUI. Developed brownish spotting for 4 days, D15, 16, 17, 18 after IUI and menstrual bleeding D18 (today)
1) Should I try another cycle of clomid at a higher dose starting tomorrow? Or wait one cycle (if HCg drawn today is negative so far pending)
2) This is first documented chemical pregnancy, how do I know if I have had previous episodes of this over the 1-2 yrs we have been trying in which case I shd get the appropriate workup? Can you differentiate chemical pg from menses based without HCG levels
3) Role of Metformin and progesterone

I can't comment on some of your questions as I'm not a doctor, but I'll answer as best I can from experience and my own research. I have found that if you respond to clomid which you did, but did not have a successful pregnancy, a higher dose of the drug will not make a difference. Your doctor will know that you responded to the clomid based on the pregnancy and ultrasound monitoring. It is also recommended that you wait a cycle after having a miscarriage, so that your body can get back to normal. There is no way to know if you've had previous chemical pregnancies. However, usually after two or three losses, the doctors will do a recurrent loss evaluation to see if there is a cause. Best of luck,
