Infertility After Multiple Pregnancies: Exploring Options & Next Steps

QUESTION: I have asked in the past with another expert..but I have been unable to get pregnant.  I have tried clomid for 3 mths now, had an hsg, had my blood checked for all the hormone and thyroid stuff...it all comes normal and the clomid hasnt helped me get pregnant.  My husband had a child from a previous relationship and his daughter is 3..my ob doesnt think its him.  Can you give me any other advise on what I can do or take?

ANSWER: Hello,

Based on what you told me, you have had normal hormones, the tubes are open and we presume that your husband is normal (based on his prior history).  So, you don't know what is keeping you from getting pregnant.   Clomid is not a magic fertility drug.  What is does is induce ovulation in women that are not ovulating or enhance ovulation (ovulate more than one egg) in women that are already ovulating.  If it has not worked thus far, then you have to look elsewhere for the problem because the Clomid is not solving it.

I would recommend a semen analysis in your husband, because time can change things.  I would recommend a hysteroscopy to evaluate your uterus and a laparoscopy to evaluate your pelvis.  I would also recommend that you go see a fertility specialist at this point to look at other treatment options.

There is something going on, you just don't know what and that is what needs to be found out.  The treatment has to treat the problem in order for you to get pregnant.

QUESTION: I forgot to mention one minor abnormality in my blood..single gene mutation of MTHFR.  Could that have anything to do with me not getting pregnant?  Could it be causes me to miscarry every mth..not allowing me to carry or continue the pregnancy for a longer period of time.  I also had a blood clot in 2001 after my second pregnancy..could these be clues to something..I am currently on folic acid and prenatal vitamins.

Hello,

A single gene disorder could lead to a reason for recurrent miscarriages since the embryo will get that gene as well.  In addition, if you had a blood clot in the vein in 2001, which you should not get at your age, then that could indicate a problem with the blood clotting system.  You may want to undergo a hematological evaluation for that.
