Fertility Concerns: Scar Tissue, Endometriosis & Natural Pregnancy Chances


Question
i'm 33-year-old female from NJ.
My left fallopian tube(shown opened on HSG) has scar tissues due to the endometriosis.
My left ovary is found to be stuck to my uterus, does this mean that the fallopian tube will never catch a mature egg from my left ovary?
i heard that there is cul-de-sac on fallopian tube? Can an egg from my left ovary be ovulated?
what are my chances of natural pregnancy?

I failed 2 IVF cycles.
1st cycle-1 immature egg
2nd cycle-1 mature egg and 2 immature eggs
I plan to try for natural IVF if I qualify. or try for another 2 rounds of conventional IVF.
What are my chances of succeeding in IVF cycle?

From your description, it sounds like you have stage IV endometriosis, which significantly decreases your chances of a natural pregnancy.  The problem is that it will be very difficult for the egg to find and get into the tube.  IVF is definitely the way to go.

I am surprised by the results of your IVF cycles.  You had two bad cycles for sure.  In your age group, you should have had a lot more eggs and embryos to work with.  For some reason, you are either not being stimulated strongly enough or your ovaries are not responding well.  I don't know which is the problem but you need to find out.  If you are not being stimulated strong enough, that is a fault of the center/doctor that is treating you.  If your getting a high protocol but your ovaries are not responding well, then that is an ovarian problem, and something that cannot be overcome.  In that case you just need to keep trying over and over.  In either case, IVF is the treatment of choice for you so you have to go in that direction.  If it is an understimulated case, you might want to look for a different center with more aggressive doctors.

I would not recommend trying a natural cycle IVF (significantly less pregnancy rates).  At your age, you should have a 50-70% chance of pregnancy per cycle, but your clinic is not getting enough eggs to work with.  They need to try to shoot for at least 10 eggs retrieved which will lead to less fertilized and even less good quality embryos.  That is why you have to start off with an adequate stimulation.  I would need to review your IVF records to give you more specific advice.
