PCOS & Fertility: Navigating Clomid, Miscarriage, and Ovulation Issues

Hi, Dr Fox.  I have PCOS and have been trying to conceive over six months.  I took Clomid in the past and I was lucky to have a beautiful son.  I started Clomid again in January and got pg right away.  Sadly, I miscarried in March.  I got the go ahead to start trying again in June, so I took Clomid again, but I had an annovulatory cycle. My temps dropped considerably about 18 days into my cycle and stayed low, and I anxiously awaited ovulation, but it never occured. In fact, if you looked at my chart, it looks the opposite of a normal chart - my temps range from 97.0-97.2 for the first part, and then 96.2-96.8 during the last part. When it's low, doesn't that indicate high estrogen?  On CD 30, I started taking Prometrium to induce menses.  In the past, when I've taken Prometrium, I take it ten days in a row, and then I start my period two days after the last pill.  I woke up this morning, after only three nights of taking Prometrium, and I'm bleeding.  Could you possibly exlain this?  I would really appreciate any insight.  I want to call my doc, but she'll just say, "Come in and see me."  I want to avoid this if possible, at least for right now.

In some cases, it is possible that your system was "resistant" to clomid this cycle. Many factors can contribute to "anovulation" or "oligovulation" which means "no" or "few" ovulatory cycle. Proper nutrition & medication may be useful in certain cases. Evaluation & testing are required to determine the best treatment. Temperature change corresponds with the progesterone which is why we see increase after ovulation(it is at this point that the ovary increases progesterone). Bleeding that you experienced can occur for many reasons & proper evaluation by your physician should easily diagnose the problem. This may include blood test, ultrasounds.. etc.
