Pregnancy Complication: Hematoma & Subchorionic Rupture - Ultrasound Timing

Thanks a lot again for your reply.
Well, my wifes stopped bleeding(no dark brown spotting even)since the last 3-4 days..she is on total bed rest.We would like to know if we can go in for another USG in a week,two weeks time? as she has already undergone 3 USGs in the 1st 8 weeks...we would like to know the status of the hematoma(size) & the subchorionic rupture!How many USGs can be done in a pregnancy?should the following USGs be transvaginal or trans abdominal?she is very wary of even doing the household chores...beleiving understandably that it could lead to further subchorionic rupture & bleeding,is this a poosiblity.Are we now more prone to pre term deliveries and placental disruption?

Thanks a lot for your prompt answer.Well,the things went exactly as you said-the doctors did an USG today they saw the embryo & heard the heart beat as well!The chorionic disruption is very minute & the hematoma is 22x8..my wife is not bleding & the doctors say the pregnancy can very well progress.We would like to know your thoughts on what would be the right lifestyle(activity,walks,socialization),food habits upto full term?Can these bleeding episodes reoccur?what should be our plan of action then?What amonut of bleeding should not be worrisome & when should be call an ambulance when it bleeds?

hi!
My wife is 5 weeks pregnant..she had severe fatigue & some occasional,mild pain in her right lower abdomen some 2 weeks ago and some spotting with mild pain in the left abdomen 2 days ago.On sonography yesterday some disruption of the foetal sac was detected,the embryo(was not seen at all)the doctors say it is too early to see the embryo and that we will repeat sonography within a week.But yesterday late evening she started bleeding(not spotting).We called the ambulance & as admitted to an hospital.On sonography there they saw a hematoma...the disruption has not increased greatly..she is currently on Progesterone,medications to stop her bleed(though the bleeding has decreased it has not ceased completely)The doctors plan to keep her in the hospital upto monday,repeat the sonography(to check the bleeding,embryo,its heart beat)
Can such a gestation last full term(with medications etc) to normal delivery?We dont mind termination of the pregnancy as we are both around 29years old.Kindly guide us as to continuing or terminating this pregnancy..I UNDERSTAND THAT THIS WILL BE YOUR PERSONAL OPINION ONLY!

Until 6-7 weeks gestation, we do not see a fetus or a fetal heartbeat on sonogram. Between 5-6 weeks gestation, we only see an empty gestational sac on sonogram with no evidence of a fetus or a fetal heartbeat. However, it is common to have some bleeding at this stage of pregnancy. Often there is noted (on sonogram) a "subchorionic hematoma" which is caused by the beginning of the development of the placenta. This hematoma usually has no effect on the growth of the baby and it will resolve on its own within a few weeks. I would not do anything. She does not have to be in the hospital unless she is so anemic and bleeding so heavily that hospitalization is warranted. There is no reason at all for a termination of the pregnancy. Just wait and see what the sonogram shows next week.

As the hematoma resolves, the bleeding will get less and less, and you should have no problems for the remainder of the pregnancy. YOu can always have more bleeding due to other reasons such as placenta previa (where the placenta covers the opening of the cervix) and abruptio placenta (where there is premature separation of the placenta). HOwever, there is no reason to suspect you will develop these problems. If you do have other episodes of bleeding, merely get into bed and refrain from heavy lifting or sex. If you do not have any bleeding, you can conduct your life during pregnancy as you do when you are not pregnant. Walk, run, jog, play tennis, go bowling, have sex (as long as you are not bleeding). Avoid sky diving or scuba diving(the baby gets the Bends). Eat anything you wish (Chinese food, Italian food, greek food, spicy food, etc.) as long as you are not having Morning Sickness any more. The nausea should go away after 16 weeks gestation. Then you can eat what you wish. Take Tylenol for pain and avoid Aspirin, Motrin, Advil, ibuprofen. Take Mylanta, Maalox, Tums, Digel, Milk of Magnesia, etc. for heartburn. Otherwise, enjoy the pregnancy. If you have any more specific questions, don't hesitate to ask.  Good Luck.

Once there is no bleeding, there is no reason to be on bed rest. She can walk around and do her regular activities. I would just refrain from sexual activity or heavy lifting until the next sonogram. You can take as many sonotrams as necessary during a pregnancy. There is no harm from sonography nor will there be any problems with taking serial sonograms. She will not have any problems later on in the pregnancy from the subchorionic hematoma, and it usually clears up within a few weeks. The sonotrams can be performed either transvaginally or trans-abdominally. Activity from now on will not cause any problems whether the subchorionic bleed stops or continues for awhile. It will not return later on in the pregnancy. Don't worry.
