PCOS, Metformin, and Progesterone Cream: Understanding Ovulation

Question
Thank you for your reply. February 21 came and went and no period. Took a pregnancy test and a negative result also had my progesterone tested and that came back showing no rise in my progesterone level. I spoke to my OBGYN and she said to wait a while longer on the medication.

I've heard about a progesterone cream you apply to your body twice a day. . . Do you have any insight on the cream. . . is it supposed to make you ovulate?

Thanks

Age: 26

Last Day of Period - Jan. 25,06 - However it was a very black discharged that lasted for 4 days.

Been on Metformin since Jan 5,2006 1500MG a day to induce ovaluation in hopes to become pregnat.

2 Questions:
1. The discharged that I had back in January not sure if that was considered my period or not?

2. Currently still waiting for my period to come. What is the usual time frame for Metformin to become effective to stimulate ovulation?

Thanks
Answer -
We usually use Clomid and Metformin for the treatment of PCOS. Metformin does not actually induce ovulation, but the comibination of Clomid and Metformin have been successful in leading to pregnancy in women with PCOS.  Always note the FIRST day of your period, not the last day of your perod. The color of the blood is not important. There is no way to know how long you need to take the medication prior to ovulating and conceiving. Just keep doing what you are doing. If your last period started on January 25, you probably were due for your next period on February 21. If yoou do not get your period by this week, take a pregnancy test. If it is negative, you are not pregnant.

Answer
If your progesterone level was low, you definitely should start progesterone supplementation. I would not  use the cream because it cannot be titrated correctly and you may use too much or too little. The progesterone wl regulate your period, but may or may not influence ovulation. I would follow the directions of your doctor. If you do not get  your period within a few wees, ask your doctor to induce your period using Provera 10mg twice a day for 5 days and you should get  your period within 10 days. Good Luck.
