PCOS, Metformin, Provera & Clomid: Early Pregnancy Experiences & Information

hi Brenda,

last jan. 18, 2008 i have been diagnosed with PCOS {polycystic ovarian syndrome } my ob said that that is the reason why im not having regular menstrual period evry month and the reason why i havent been pregnant for a long time.

my daughter is now 7 yrs old and my hubby and i are trying to conceive for the past years. so i was advised to take metformin , my LMP is jan, 14-18 2008 so  i was advised to take provera on feb 15, 2008 if i dont have menses by then. so on the night of feb 15 i startd taking provera 1 tablet a day for 7 days. i was advised that w/in a week i shld have menses, on feb. 21 exactly 7th day i had menses but just for that day, just one day and it is lighter than regular menses..so i called my ob and i told her that im wondering why i only had menses for one day? ob said its ok and so i asked if i still should take clomid on feb 23 as i was advised to take clomid on 3rd day of menses, my ob said yes,, so i took clomid for 5 days 1 tab a day, then my  scheduled follicle monitoring came,,,march 3, 2008 that's when i found out that i am already 5 weeks pregnant.  i am worried as the meds i took ,metformin, povera and clomid might affect the baby as based on calculation i am already pregnant when i took those meds . during transv ultrasound no fetal pole seen yet , only gestational sac so i was advised to have ultrasound after 2 weeks for definitive aging and viability. then march 8, 2008 i had vaginal bleeding and cramping. i was confined because my cervix is open and it was diagnosed as threatened abortion, so i was put on IV with isoxilan for 4 days and when i got home i was instructed to take duvadilan 3 times  a day for 7 days and now evrything is fine, baby is ok and no bleeding at all.. my question is does provera, clomid and metformin triggered the bleeding and does those meds has an effect on the baby ive read some articles that it can cause birth defects,,,please help me understand.i am worried  thanks in advance for your answer

If the ultrasound on 3/3 determined you were at 5 weeks gestation, that would calculate back to approximately a 2/11 conception date. (All pregnancy dating is based on LMP, which is assumed to be two weeks before the conception date) So, the Provera and the Clomid would be the only medications which were taken after 2/11.

The Provera would not be anything to be alarmed about. Many doctors are prescribing Provera (synthetic progesterone) for pregnant women whose progesterone levels are lower than we would like anyway in an attempt to maintain the pregnancy.

Because it would be essentially unnecessary to give Clomid to pregnant women, the use of Clomid in pregnant women has never been studied in a clinical trial. However, based on animal studies, we know about the problems that Clomid may potentially cause. Therefore, Clomid is not recommended to be given during pregnancy because of its risk of birth defects.

However, there is a fairly common theory amongst health care providers called the "all-or-nothing phenomenon". Essentially this refers to a window of time in the first 2-3 weeks after conception when there is none of the crucial fetal development going on. During this window, if the new fetus is exposed to anything which will prove to be "defective", the strong likelihood is that it will spontaneously abort. However, in most cases, this window allows for great latitude. My personal suspicion is it was a nature-made, divine protection since most women will not know they are pregnant for at least 2-3 weeks after conception has occurred.
