Understanding hCG Levels and Miscarriage Concerns: A Doctor's Explanation

I found out that i was pregnant on Sunday (Easter) and my bchg level was at 540 and now there are dropping. My Dr. thinks that i am miscarrying. Which i would agree, but heres is my confusion My levels are dropping slowly, I have very light too no bleeding my cervix is opening i have no blood clotting and he says he thinks it tubal but i was at the hospital and the ultrasound shows a sac in the uterus. So i don't know that i am miscarrying. I have had abdomen pain. Could it be possible that I have twins and one in my tubes and one my uterus. and the one in the tube is miscarrying or visaversa????

Usually with twin pregnancies, the hCG level is higher than normal to begin with. Also, an ectopic (tubal) pregnancy SHOULD have been spotted with ultrasound, even if a sac was seen in the uterus. They should have visualized all reproductive organs at that time. However, you know what they say about assuming...

If your levels are falling, that is not a positive sign. We expect hCG levels to double every 48-72 hours in the 1st trimester of a healthy pregnancy. So, no matter the speed at which they are falling, the fact that they are falling is not encouraging. I would recommend that you stay aware of any signs of miscarriage - Brown or especially bright red bleeding with cramps, tissue with clot-like material passing from the vagina, sudden decrease in signs of pregnancy, or mild to severe back pain (often worse than normal menstrual cramps). If you happen to have any sharp or stabbing pain (may come and go and vary in intensity) in the pelvis or abdomen (especially on just one side) or even the shoulder and neck this can be due to blood from a ruptured ectopic pregnancy gathering up under the diaphragm. At that point you should seek medical care right away.
