Understanding Fetal Demise: A Personal Experience and Questions

Question
I recently experienced a fetal demise at 24m6d preg.  This is my first pregnancy- 24 yrs old- I was diagnosed w/a bicornuate uterus @ 8 wks pregnant when I experienced a significant amount of bleeding- my ob didn't feel that this was a contributing factor to the demise-nor was I monitored any more frequently in regards to the condition.  All of my ultrasounds were normal, fhr was within normal range, anatomy scan was "perfect" they said.  The OB stated that my uterus was expanding normally and that the bicornuate factor was not an issue...I went in to the ED because I had an absense of fetal movement.  Nonetheless, there was no heartbeat.  I was wondering what factors/if any could have made this occur-the placental culture and all my labs came back "normal" or within normal limits. I wouldn't think there could not be a reason for this to occur- Any advice/information would be greatly appreciated.  Thanks a lot in advance.

Answer
Hi Stephanie,

I'm sorry for your loss. Unfortunately sometimes there is no reason for fetal demise, it is just unexplained which I believe is the case for you. Infection is a high risk for the fetus. Bacterial infections where bacteria enters through the cervix into the amniotic fluid triggers a cytokine cascade that ultimately leads to fetal damage and fetal demise. If a knot in the umbilical cord occurs then there can be constriction of the blood vessels and fetal circulation can not be maintained.
