Understanding Fetal Age & Early Pregnancy Concerns: A Doctor's Perspective

Question
I do not ovulate - but with clomid I was able to conceive. The problem is b/c no one knows when I ovulated how far along I am. I heard anywhere from 7.5 weeks to 5.5 based on the size of the embryo. The doctor said everything looks good (gestational sac, yolk sac, etc.) She was however, "cautiously optimistic" but was concerned about the embryo size (2mm) - but originally I was supposed to be further along. Should I be worried that the embryo is too small at 5.5+- weeks (which is what the sonogram said I was based on size of embryo)?

Answer
Dear Rebecca,

Because it's essentially a guessing game as to when you conceived, it will be difficult to know how well the fetus is developing in these early stages or whether all is "on schedule".

The earliest CRL/gestational age comparison chart that I have is for 6 weeks. At 6 weeks gestation, the average CRL measurement is .45cm or 4.5mm. Now, this may seem to be a huge difference to you, but at 6.5 weeks, the average is 6.5mm. So, your CRL measurement may not be anything to be alarmed about. It may only mean the difference of 4-5 days in a conception date.

I'm sure that your doctor will do fairly frequent ultrasounds in these first several weeks. A follow-up ultrasound in another few weeks will help to further determine whether dating is accurate and ease you and your doctor's minds as to fetal growth.

I hope this information has helped you and answered your question. I wish you well and congratulations!
