Understanding Low Segment Count & Anion Gap: A Medical Explanation

Dear Samantha:

I received my lab test values in the mail and everything was within normal limits except the seg count which was 38 (range is 45-77) and my anion gap, which was 7 (range is 8-16).  I am concerned about these low readings.  As a helpful marker, my albumin was also relatively low but normal (3.8)  I have been under recent stress lately and have been exercising strenuously, but otherwise am fine.  I do not drink or smoke, am 44 years old and in relatively good health.  Please tell me what you think and if I should follow up on this._

Hi Barbara, I'd be glad to try and answer your question.  The seg count is a measure of one type of white blood cell called a "segmented neutrophil".  Yours is only slightly low at 38%.  I don't think you have anything to worry about with this, since the other portions of your CBC (complete blood count) are within normal limits (I assume they are).  Also the anion gap is a calculation using four chemistry tests, sodium, potassium, chloride, and bicarbonate.  As long as these four tests are within normal limits, then I wouldn't worry.  If all of your other tests are within normal limits as you say they are, I wouldn't worry.  I don't believe a followup is necessary.
