Understanding Post-Chemical Pregnancy: What to Expect & Next Steps

Question
Hi,

I'm just writing to ask for professional advise as I'm so worried and frustrated. I'm 24 years old and married few months ago. I'm actively trying to get pregnant but no luck till now.

I was diagnosed with PCOS one year ago but I took birth control pills for 3 months and my period was very regular since then. (and remain regular for at least half year). My cycle is 31 days

My last period was 14th -18th July.

On the 31st July, I have a dark line in LH paper(ovulation test) and we had sex on that night. And the line in LH paper disappeared since then.

On the 9th Aug (9 days after ovulation), I had a brown spotting (which I assume to be implantation bleed). And I have a faint line in LH paper.

On 10th Aug, I have nausea for the whole day and about 50% in LH paper.

On 11th Aug, the LH paper gone up to 100% and I have a faint line in HCG paper (pregnancy test!).

However, on the 12th  Aug, all those sickness gone and the HCG paper turns to be blank again.

I assume it is a chemical pregnancy , do you agree?

It's OK that I have pregnancy, I was upset for a week but I'm ready for the next cycle. However, my period never arrived till today (21 Sept).

I feel very annoyed by it. I did have some very light brown spotting between but it is very very light that hardly considered as a period. Also, my LH paper is always has two dark lines since 11th August. I had a HCG blood test 3 weeks ago so I'm pretty sure I'm not pregnant. But why I still don't have my period? And why my LH paper always showing two dark lines? Does it mean I'm having the PCOS again? When can I expect my period again? If I really getting the PCOS again, is there anything I can do to increase my chance to get pregnant?

Thanks a lot for your help. I did see a GP before and ask my concerns to him. However he didn't seem to understand me and just ask me to wait for my period and there's nothing he can do he said.

Answer
Hello Katie from Australia,

It is certainly possible that you had a chemical pregnancy, but I am not confident enough with urine pregnancy tests to draw that conclusion.  It would have been better to have a blood test giving a specific serum level of bHCG.

I'm afraid I can't explain the phenomenon with the LH test lasting this long.  It is probably getting cross contaminated with some other elevated hormone (again, I do not use these tests because of their unreliability).  Sometimes if you have a persistent corpus luteum cyst or other type of physiologic cyst, they could be hormone producing and will prevent your period from being normal and possibly give you the false LH readings.  You would need an ultrasound to detect that.

At this point, we can certainly draw the conclusion that your cycle is abnormal. What needs to be done now is to get it corrected.  I would recommend that you see a Gyn doctor to have that done.  It may take using a medication such as Provera or birth control pills to get your period so that your ovaries can re-set.  It may be a recurrence of PCO, but it is too early to make that conclusion.

Good Luck,
