Medical Examination Form Templates & Resources - For Non-Physicians

Hello, i'm searching for forms that constitute a general examination for each of the major fields of medicine; ent, vascular, pediatric, etc.

what i need is content for templates to serve as records for individual patients at specialized clinics. i'm not an MD and would like to be just guided to where i can find this information.
