Answerthere are never any plans that don't use standard claim forms; you are asking for something that doesn't exist. However, we are looking into it now, but there is a new trend to pay an "administrative fee" yearly to the doctor, then your care for the year is covered, you can go and be seen same day for illesses, have access to your own doctor, but it will be pricey, somewhere around $1000/year which will not cover hospitalization. This has been tried in some areas, like Seattle and Chicago; but all insurance companies need their forms......
