Persistent Fever & Unusual Symptoms: Should You Seek Further Medical Advice?

Question
Hi, I've got something weird going on and I've seen a bunch of doctors but I want advice on if you think I should go again.

I'm a 25 year old female, hypothyroidism under control, and chronic stomach/bowl issues.  (IBS, heartburn, etc).  I take meds for the thyroid, stomach, and allergies.  There is no way I could be pregnant.

Last Thursday night (a week ago) I had a low fever.  I also had intense muscle pain in my shoulder and a wierd spot below my left ribs that also felt like muscle pain and was agitated with the same things that bothered my shoulder.  I went to Urgent Care on Saturday.  They did blood work (white count was high, something was high that indicated I had inflamation).  They sent me to the ER, where they did a CT scan and an EKG.  I guess nothing showed up because they ER doc sent me home with two different pain meds and a naproxin prescription.  He also told me to go to a doctor on Monday.

On the advice of the pharmacist, I filled only the weaker pain meds.  I have a history of problems with my stomach and he thought the naproxin might be bad for it.  The pain meds made me feel weird, so I stopped taking them.  I had a low fever (never over 100.7) all weekend.  I also still had pretty bad pain.

Monday I went to the doctor, and did NOT have a fever.  That doctor advised me to fill the naproxin, so I did.  The naproxin helped the pain.  Tuesday I did not have a fever either, so I went back to work.  Tuesday evening I developed a low fever (99.9).  Wednesday morning it was back at 100.4 so I went back to the doctor.  I also had developed a stuffy nose.  That doctor gave me azithromicin.  So I've been taking both that and the naproxin steadily.  I never missed a dose of either.  The fever was gone Wednesday night.

Thursday I woke up with my highest fever yet.  101.3,  So I called the doctor and they said just keep taking the meds.  I also took tylenol, and pepto bismol because the z-pack gave me diarrhea.  The tylenol didn't really do anything because I took it at 10:30, and I still had a fever at 1:40.  (100.8)  However, by that evening the fever was gone, although I took no more tylenol.

This morning I woke up and my temp was 99.7.  I thought good, we're on the right track.  However, just a half hour ago, I felt a lot worse so I took it and now it's 101.8.  That's the highest yet, again!

Do you have any idea if this is normal?  The doctor wasn't really sure what I had when he gave the z-pack.  He said either sinus, strep, or possibly pnuemonia.  I've never had trouble breathing.  When the muscle pain was bad it hurt to breathe deeply, but I could.  At what point does this stop being weird and become something scary?  At what point should I expect to feel better?  I only have two more z-pack pills left.  Should I just wait it out or try to see another doctor?

Thanks for your help,
Beth

Answer
I assume you had your urine checked for a UTI (urinary tract infection).  This sounds like you had a slowly progressing viral upper respiratory infection.  Most of these simply go away, some progress to a full bacterial sinus infection that may need antibiotics.

I would finish the zpak.  If you have sinus pain/pressure/nasal congestion, use some AFRIN NASAL SPRAY twice a day for 3-5 days only.  This time frame isn't unusual... I would give it a few more days.
