QuestionQUESTION: Doctor,
I was about 4 weeks pregnant when I got my tests done. I checked to see if I was pregnant both via a home pregnancy test kit and by getting an ultrasound. The radiologist wasn't sure if the foetus was still alive as it didn't have a heartbeat. He recommended I wait seven days and check to see if I'm still pregnant. I used a test strip to check and turned out I was still pregnant.
So I decided to do a cycle of Mifepristone (200 gm, taken orally) and Misoprotol (200 gm, 4 tablets, taken vaginally). But before I could take misoprostol, I started bleeding. Do I continue the dosage and take the misoprostol? The problem is that I have been spotting erratically for a day or two, once or twice a day, in the past week. But the test strips continued to show I was pregnant. I'm sort of confused now as how to proceed. Kindly help.
ANSWER: Dear Aarti, YES.
Everybody will bleed after mifepristone.
You need the misoprostol also to be absolutely sure.
Good luck.
Dr Shan
PS: Pregtest stay positive even 3 weeks after an abortion.

QUESTION: Hi,
Thanks for the prompt response. I was wondering if te second pill, Misoprotol can be taken orally. Ive done some research online and several websites suggest that it may be taken orally (bucally or lingually) as long as the pill is not swallowed for at least 30 minutes. is that correct? the drug I purchased gives no such information in the packaging.
Is it safe for me just place the pill in between my cheek and gum (bucal region) and keep it there for 30 mins? I'm also very confused about the dosage. The kit came with 4 tablets of 200 gm each. Do I take them one after the other? If not, how long do you have to wait before you can take the second pill etc.?
Thanks for your help.
Have a good one.
Aarti.
ANSWER: Hi Aarti again,
yes, you might take those pills bucally.
No problem. 4 pills at once is OK. Two in the right cheek, two in the left.
I do not understand the advise of the radiologist. At four weeks there is never a heartbeat to been seen.
you wrote: " I used a test strip to check and turned out I was still pregnant. "
This makes no sense. Pregtests stay positive upto 3 weeks after the abortion.
bye for now.

QUESTION: Dr Shan,
Well, he kept insisting that all he could see was a foetal sac and he wasn't
quite sure if it was alive anyway. So he told me to wait 7 seven days and get
another ultrasound. But before I could do that, I started spotting erratically. I
figured I was going to get my period in a day or two. I waited about 2 weeks,
didn't get my period, so I went on and did another test. That came out
positive. I didn't know that those are positive even after 3 weeks of abortion.
Either way, since I wasn't getting my period (I waited for like 3 weeks total
since I got the ultrasound) before taking the pill. I took the mifepristone
orally and waited a day before taking the misoprotol. I took that bucally but I
only took 2, one on each cheek to start with. Within an hour I already had
massive cramping in my abdomen and heavy bleeding. I'm not sure if I
should take the other 2 pills now or wait for 3 hours as some websites
suggest.
AnswerHi Aarti again,
You did OK. 2 tabs of miso is sufficient in your case.
I think the abortion was complete already, and coming out after the miso was only some debris and blood.
It certainly was NOT your period yet.
Periods after an abortion are unpredictable and irregular.
So, no worry, its just patience you need (and no unprotected sex !)
be happy,
Dr Shan
