Pregnancy Test Positive, Ultrasound Negative: Possible Explanations & What to Do


Question
My girlfriend is 31yrs old, her first day of her last menstrual period was March 14,2004.

Question: Is it possible to have a positive result from the pregnancy test kit, but in the ultra sound having a TVS Ultrasound the result is negative? Up until now she haven't had her period.

The result if i may share:

The uterus is normal in size 6.7X5.2X5.1cms.(LWH).Uterine echoes are homogenous with no evident focal lesion seen.
Endometrial stripes is thickened measuring 15.2mm. Minimal fluid is noted along the upper endometrial cavity. No demonstrable intrauterine gestational sac nor embyonic pole.
Both ovaries are unremarkable. The right ovary measures 2.6X1.5cms., while the left ovary measures 2.9X2.2cms. No adnexal mass seen.

REMARKS:

Normal sized uterus with thickened endometrial lining and minimal fluid at the upper endometrial cavity. Unremarkable ovaries ultrasonically.

Answer
If a pregnancy test was positive and a sonogram did not show any intrauterine pregnancy, or any abnormal adnexal masses, either the pregnancy test was incorrectly done or was in error, or the pregnancy is too early to show anything on sonogram. However, if the last period started on March 14, she is only 5 weeks 2 days pregnant. A sonogram will not show anything except, perhaps, an empty gestational sac, but may not even show that yet. I would do two things. Take two blood tests for pregnancy (Beta HCG) 48 hours apart. If there is a doubling of the results in 48 hours, it indicates a viable pregnancy. Then, repeat the sonogram when she is 6-7 weeks from the first day of her last menstrual period. At that time, there should be a gestational sac, a fetus, and a fetal heartbeat. Another reaason for a positive pregnancy test and a negative sonogram is the possibility of a miscarriage, and one can never rule out an ectopic gestation. However, it is a bit too early to tell anything at this time.
