Early Pregnancy Bleeding & Pain: What Could It Mean?


Question
So basically you think I am not pregnant? I also thought that as well, I mean i have all the signs of having a miscarriage. Well, we will find out the results of the blood test on monday. If I have had ANOTHER miscarriage do you think there may be something wrong with me? I mean I had a still birth at 7 mths and a miscarriage. Do you think I should have numerous tests done to find out if I there is something wrong with me. I mean will continue having these same problems?

Hello thanks for answering me but I took a blood test and it said I was only 1 week. anyway, I went to the ER today because I was bleeding really bad and pretty bad cramps, the urine test said I wasn't pregnant but the blood test said it was positive. They said my BHCG is low it is about 2.something. What does this mean? They said I might be endanger of having an ectopic pregnancy or it may just be a normal pregnancy. What should I do now? Should I go to get blood work tomorrow or waite till 2 days and when should I get my ultra sound? What do you think it is?

I am 19 years old and I have no children, my last period was around Oct. 20th. And today is November 23rd. This week I found out that I was pregnant one week pregnant. Before this pregnancy in August 26th I was 7 mths pregnant and had a still birth. And now I am pregnant again. I have just begun bleeding this afternoon as if I was just starting my period it is a light pinkish red color but light not really heavy but I also have lower abdominal cramps. Not reallly extreme, but enough to feel like a period. I am very worried because of my loss previously. I called my doctors office they told me to rest and if it bleeds severly and I have bad abd. pains to come into the emergency room. So should I be worrying or is this normal or could it already be a  miscarriage? what should I do? Please let me know as soon as possible.

Miscarriages occur in 15-20% of every pregnancy. There is nothing you can do to cause a miscarriage and there is nothing you can do to prevent a miscarriage. An early miscarriage is usually due to some problem in the meeting of the sperm and the ovum. A miscarriage is in no way connnected to the fetal demise you had at 7 months, last time. If your last period started on October 20,  you are now 5 weeks pregnant(from the first day of your last period). It is common to have some bleeding and cramps as long as it is not heavy bleeding or severe cramps. A sonogram at 5-6 weeks gestation will only show an empty gestational sac, but will not show any evidence of a fetus or a fetal heartbeat yet. By 6-7 weeks gestation, a sonogram will show a fetus and a fetal heartbeat. A blood test for pregnancy taken twice, 48 hours apart, should show a doubling of the result in 48 hours. This indicates a viable pregnancy. However, a halving of the results in 48 hours indicates a miscarriage. If you are not having heavy bleeding or bad pains, wait another week and take the blood test for pregnancy as well as a sonogram and you will know what is going on. The best time to take the next sonogram will be December 2. Of course, if you have heavy bleeding and severe cramping, go to the ER right away. In answer to your queston, this can be normal or can indicate a "threatened miscarriage", but there is no way to know for sure at this time. Be patient.

If your last period started on October 20, you are 5 weeks pregnant. Blood tests will not indicate how far pregnant you are. They only give a range of HCG values compared to weeks gestation. Once you miss your period (14 days after conception), HCG values range from 9-130 (3-4 weeks gestation), at 4-5 weeks gestation, the values range from 75-2600), at 5-6 weeks gestation, the values range from 850-20,800, and at 6-7 weeks gestation the values range from 4000-100,200.  If you only got a value of "2", that indicates that you are not pregnant. If you take the test again in 48 hours, if it is still less than 5 you are probably not pregnant. Sometimes there can be an error in the qualatative blood test compared to the quantitative blood test. A sonogram at this time will not show anything, as I explained before.   Once you have reached 6 weeks from the first day of your last period, a sonogram should show a fetus and a fetal heartbeat. If not, you are not pregnant or are too early to tell.

As I mentioned before, the risk of miscarriage is 15-20% of every pregnancy for every woman. There is nothing you can do to cause a miscarriage and there is nothing to do to prevent a miscarriage. The stillborn you had at 7 months was not a miscarriage, but a fetal demise and there are many different causes for a fetal demise (infection, trauma, catastrophic events, placental insufficiency, abruption of the placenta,incompetent cervix, etc). You had one miscarriage and it appears that you are having another one. There are no tests that can be done to determine why it occurred and it is doubtful that you will have another miscarriage. Sometimes, after 3 miscarriages, your doctor can perform tests for antiphospholipid antibodies, cardiolipin antibodies, Lupus antibodies, and other autoimmune problems, but most of the time these prove futile. Statistics are on your side and I would suggest that if you do, indeed, miscarry this time,  wait until you have a normal period and then start trying to conceive right away. Don't feel doomed. I have many patients who had 3 miscarriages in a row and then went on to have 6 normal healthy babies in a row. I have other patients who had several babies followed by several miscarriages, and I have some patients who alternate between a miscarriage and a normal delivery. There is nothing you can do to change the course.
