Lovenox and Pregnancy: Risks & Considerations for Heart Conditions

Question
I am 38 years old and seven months pregnant with a baby boy.  I have a little girl who is almost 3 years old.  I have a complicated history which has taken on a few new issues with this pregnancy.  I have had a heart catherization and a heart ablation in 2000 (history of tachacardia, minor heart attack yet now I am "fixed" and feel fine - no heart disease), two simultaneous pulmonary embollisms and pericarditis - most likely a result of ablation, I'm ANA positive and possibly have lupus (but I doubt it).  You'd never know, I am healthy and physically active.  With this pregnancy (our 2nd "miracle")I was diagnosed with Factor V Leiden and my baby has a single umbilical artery.  As a result, I agreed to an amnio - I had to have TWO due to an anomoly on my 14th chromosone on the first.  My cultures were slow growing but the second amnio - after 17 days, showed a perfect baby.  He is also growing and the last sonogram showed him to be in the 65th percentile.  There is my history - here is my question.  I am on Lovenox - 40mg single daily injection, I am taking iron for anemia, I have a "heart shaped uterus", he is a SUA baby and my little girl came three weeks early and like a rocket (perfectly healthy at 5.04 lbs.)  I imagine this one will come early and I hate to admit it but I am terrified that I will "bleed out" since it would be impossible to know exactly when I will go into labor.  Should I get family to donate blood, how will they thicken up my blood safely?  Although I am seeing a high risk Dr. as well as a group of perinatoligists, I haven't gotten a straight answer that I feel comfortable with regarding my concern of a safe delivery.  Please help me put my mind at ease.

Answer
Due to your previous history as well as your current blood dyscrasias, it is prudent to continue the Lovenox throughout the pregnancy. We have had much success with the use of Lovenox and you should not have any problems due to its use. Don't worry about blood transrusion requirements. If they will be necessary, they can be arranged almost immediately. You appear to be doing fine this pregnancy and there is no reason to suspect that you will have any problems at the time of delivery.
