Amniotic Fluid Leakage at 21-24 Weeks: Risks and Monitoring

Question
I am 32yrs old and this is my first pregnancy.  My LMP 9/2/05. My due date is June 9, 2006

I started to leak amniotic fluid at 21weeks.  At that time the dr. did and ultrasound and my AFI was greater than 9cm but less than 10cm. At 24 weeks my AFI decreased to greater than 5cm but less than 6cm.  I am scheduled to check into the hospital at 26weeks and stay there for the duration to be monitored for infection and fetal distress.  What are my chances of infection at home for the next two weeks and how do I help increase my amniotic fluid.  I've been drinking an average of 8-16oz glasses of water a day. Plus Milk and Juice.  My biggest concern, is there enough Amniotic fluid and risk of infection.  I know my chances are high for premature labor and my goal is 30+ weeks. Help me understand how amniotic fluid is developed and what is dangerous AFI.

Thanks Jean

Answer
Amniotic fluid is produced by the kidneys of the baby and is urinated into the amniotic sac and swallowed by the fetus. Drinking  juice or water will not increase the amniotic fluid. An amniotic fluid index of less than 8 is worrisome. You can devedlop an infection due to leakage of amniotic fluid, due to the fact that there is an opening somewhere to the outside and an infection can "ascend" to the uterus and you can develop chorioamnionitis.  Until you are admitted to the hospital, take your temperature frequently and, if you develop a temperature of over 100.4 degrees F., go to the hospital immediately. Once you are in the hospital, they will take serial CBCs as well as your temperature. If there is any rason  to deliver you early, they will. HOpefully, you can continue the pregnancy as long as possible to increase the chances of maturity of the baby's lungs. Good Luck.
