Gestational Sac Size & Paternity Concerns at 27 Weeks: A Medical Discussion

QUESTION: Actually I have had my HCG levels taken already. I am afraid I have not communicated with you very well. I really am not too concerned with the viability of my pregnancy. I am currently 27 weeks pregnant. The reason I asked about the gestational sac size is because I am having an issue of paternity. Let me explain. My LMP was June 7th. I had unprotected sex with my current partner the whole month of June, on average every other day. I made a huge mistake and also had sex with my ex on July the 3rd. Although it was not protected, he did pull out prior to ejaculation( way before). My cycles are 30-33 days, and are regular. I am fairly sure I had implantation bleeding on the 5th of July. I was also beginning to experience pregnancy symptoms the same week I was with my ex( actually the same day). I had a very dark line on a pregnancy test on July 10th. I had my HcG done on July 14th and my levels were 1940. I had an ultrasound on July 16th that measured me at 4weeks 5 days with a gestational sac measurement of 8mm. And I had an ultrasound on September 11th that measured me at 13 weeks 2 days. As you can imagine, I am having an extreme amount of anxiety about this. I guess the question I really want to know is, do you thing that my HCG level could be that high 11 DPO, and do you think that both ultrasounds could be wrong??? Also, with 30-33 day cycles, and my last period being on June 7th, do you think I would even be ovulating on the 3rd of July. Thank you so much for your time and patience.

ANSWER: Dear megan

Now I understand your fears. No one can ever give you an answer which is 100%. However, some points may favor that the father is your current partenr:

1-Cosidering your cycle is 30-33 days and your LMP was on 6/7, then ovulation probably occured between day 16-19 of the cycle. Adding this to your LMP on 6/7/07 means that ovulation occured between june 23 and june 26. Consider the duration of the journey from the distal tube to uterus to be implanted if 6-12 days, this means implantation bleeding may occur june 29 till july 8. you already describe implantation bleeding on July 5.

2- Serial ultrasound are usually accurate. you had 2 ultrasound dating your pregnancy to early june and this coincides with your LMP

3- your ex had withdraw way before ejaculation

All 3 points suggest that it is hard for your ex to be the father. However; as i mentioned earlier, no 1 can ever garantee. A DNA test is the single test 2 confirm paternity.

ANSWER: Dear Megan

I think your question is: do you think it would even be possible for me to get pregnant 4-5 days before my -rather than your- period?

Pregnancy duration is counted from the first day of the last menstrual period; a time at which, in realty, pregnancy does not occur. Pregnancy occur almost 2 weeks later around the time of ovulation which is almost 2 weeks before your first missed menstrual period.

In fact, the duration of pregnancy is around 38 weeks calculated from the date of ovulation. Becuase the date of ovulation cannot be determined retrograde when a pregnant women present at clinic for antenatal care, we calculate the duration of pregnancy, as 40 weeks, starting from the first day of last menstrual period becuase this date should be known to all women.

Therefore the answer 2 your question would be..yes pregnancy normally happens up to 15 days before your first missed period.

QUESTION: Ummmmm...it kind of answers my question. Let me clarify what I mean. I have read many times that a luteal phase has to be at least 10 days to support pregnancy. So lets just say my period is due on Jan 4th. Would it be possible for me to get conceive lets say today? I just always thought that would not be possible because there would not be enough time for the egg to fertilize, travel down the fallopian tube, implant, and release the hormones that tell you body not to shed its uterine lining.

In any normal female who had a pregnancy or who is pregnant there is adequate luteal phase. This phase is roughly of 14 days duration in all normal women. This means a woman with 28 days cycle will have 14 days will have roughly 4 days menstruation, 10 days proliferative phase and 14 for luteal phase. In your case of 33 days, you may have 5 days menstruation, 14 proliferative and 14 for luteal phase. Luteal phase occurs just after the release of ovum. That is day 14.

Consider the life span of an ovum 24 hours and that of a sperm 96 hours (4 days)..if u ovulate and had sex at the same day,e.g. day 14, then u may get pregnant for the next 24 hours (the ovum lives only 1 day). On the other hand if u had sex on day 11 and u ovulate day 14, u can get pregnant immeditely or up to day 15 because the life span of the sperm is 4 days and for ovum only 1 day.
