Pregnancy Dating Discrepancy: Understanding Uterine Measurements & Ultrasound Dates

Question
Hi Brenda,
I nee some help. On Feb 13th I went to the doctor and he measured my uterus at 11 weeks and also we heard the baby's heartbeat. on Feb 19th I went for a ultrasound and they measured my baby at only 10 weeks 0 days. So where I am confused is that my last period was in November and I wassn't sexually active from Dec 9th through Jan 1. so all the calculators that i've been looking up have shown me to be 12 weeks at the time of ultrasound. also when i went to my doctor appt this month on the 13th of march my doc said that he could feel the top of my uterua and that usually happens at 14-15 weeks. but the nurse was calculating me at 13 weeks due to my ultrasound. I cam so confused and really need some help. I did have un-protected sex one time on dec 26th but i took Plan B about 6-7 hours after.
I am so confused and really don't know what to do. I am hoping i concieved in the beginning of december and that my dates are off from my ultrasound. the numbers on my ultrasound are:
FRQ 8.0 MHZ
GN 34
SA 2/3
D 6.0 CM
DR 72
FR 11 Hz
AO 69%

I have no idea what these mean. Can you please help me figure this out? I know that Plan B is only 89% effective. but i did take it soon and had no other contact that day so i'm hoping i didn't get pregnant on that day.
I weigh about 184 LBS when i heard my baby's heartbeat at the doctor's office on Feb 13th. When according to the ultrasound done the following week i should have only been 9 weeks. I heard it through that dropper the doctor uses.

Answer
Dear Karen,

Because I am not a sonographer, I am sorry to tell you I have no idea what the codes mean either!  :-)  Sometimes I can decipher - these I cannot.

Regardless, of all of the information you have given me, I tend to trust the ultrasound results the most. Here is why: Ultrasounds done within the first trimester are considered to be very accurate in dating pregnancies. Since having the ability to visualize pregnancies via ultrasound, we have noted certain "norms" at specific gestational ages in the early weeks of pregnancies. So, by measuring the gestational sac and CRL (crown-rump length) and also noting the appearance of certain things, such as the yolk sac or heart activity, we can date pregnancies quite accurately to within a few days. The biometric crown-rump measurement (CRL) of the six to 12-week fetus provides the most accurate measurement - within three days of true gestational age. From 12 to 18 weeks gestation, other anatomic structures are typically measured and biometric measurements are usually accurate to within a week of gestational age. After 26 weeks gestation, fetal variability has greatly increased and the dating provided by biometric measurement is less accurate, but the method is still used. By late in the third trimester, measurements are accurate only within a two-to three-week window.

So -- the 10 week ultrasound would/should have been the most accurate tool in dating the pregnancy (to within about 3 days of true gestational age). If you had that ultrasound on 2/19 and they measured right at 10 weeks, I would calculate that the estimated conception date was on/about 12/25/08. Also... by that exact same calculation, you would be 15 weeks today and due 9/17. That makes the fundal assessment at the doctor's office this week make sense.

I hope this information has helped you and answered your question. I wish you well.

Brenda
