Early Pregnancy Symptoms & Concerns: A Patient's Experience

I m very much upset, I don't know what's going on with me. My last LMP was on 7th may'09, have a history of irregular period since last 9-10 months. I took the PG Home test on 12th june it came positive, after two days also it I again rechecked at home it was again positive. But on 14th May I was feeling little cramping with brown spots, though it was minor. I cntacted my GYN she gave me 2 injections Chromostek and Maintain 500, after wards in 1-2 days everything comeback to normal. She told me to take complete bed rest for a week, I followed same also. Again I have done lab test it came weak positive, afterwards on 20th June i repeated again my LAB urine test, it came positive. On 19th I was feeling some pain like usually I feel during my periods, i contacted my GYN she again gave me same injection but qty is 2nos and that too directly in my VEIN. After 1-2 days of light spotting everything came back to normal. I again visited my GYN she told me to have u/s, I done the same on 24th June, but it was not showing any thing. I was so depressed my GYn told me I have lost my baby, but i haven't lost the hope. On 1st july I had some clotting around 2-3 pcs of about 1"inch and I bleed that day, means I need to use my pad. After 1 days again I back to normal. My GNtold me to do B-HCG, I done on 4th July, result was 2377.4 miu. My GYN told it shows I m pregnant, she told me to have u/s again i did on 6th July in this as per them no sac, in Left ovary it shows a dominant follicle of 22mm. My GYN has told me to repeat the same B-HCG and u/s... Can u pls help what's gping on. I m prgenant or not?? Is is possible i had m/c, whaqt is the possibility of this pregnancy. Believe me I will be thankful to you throughtout the my life for this help.

It is very difficult for me to answer your question. First - I am not familiar with the medications used. These are not medications that are used in the U.S. Typically, there is not alot that can be done when a miscarriage is imminent in the 1st trimester. There is often a reason for it, the pregnancy is not healthy or there is a problem with the fertilized egg that the woman's body recognizes and nature simply takes over and expels it.

The vaginal bleeding alone is not a symptom that would absolutely indicate that a miscarriage has occurred. Many women experience bleeding during the first trimester of pregnancy and go on to deliver healthy babies. But because I'm not able to know what your hCG level was before the amount of 2377 mIU/ml, I cannot know whether the hCG level is still rising or if it is falling. That would be the biggest indicator of a failing/failed pregnancy. I can tell you this so you know it when you go back for your follow-up level - the hCG level is expected to double every 48-72 hours in a healthy pregnancy. Therefore, if you return for a repeat level on 7/6-7, your hCG level should be at least 4754 mIU/ml. On 7/8-9, it should be 9508 and so on. If those levels are not rising appropriately or are falling, I unfortunately would not be optimistic about this particular pregnancy.

I will also tell you that if your LMP was 5-7-09, you would have been 8 weeks along on 7-4-09. At 7-8 weeks of pregnancy, the average hCG level is 7,650-229,000 mIU/ml. So, yours would be very low for the stage of pregnancy you should be in. Also, at 8 weeks of pregnancy, we expect to not only see a gestational sac, but much more. The gestational sac can typically be visualized as early as 4.5 weeks of gestation and the yolk sac at about 5 weeks. The embryo can usually be observed and measured by about 5.5 weeks. A visible heartbeat can often be seen and detected by ultrasound at about 6 weeks and is usually clearly detectable by 7 weeks.
