Follicular Cyst & Ovulation Tracking: A Personal Experience

QUESTION: My previous cycle I had a follicular cyst that was 33x44mm so I went on birthcontrol pills for 21 days then on cd 24 the cyst was still there but measured 13x8mm.( Md said that I could o around this)
So I stopped BCP and started my period the next day and then took femera cd 2-6.
On Cd 14 I had 3 follicles measuring 30x33mm,21x24mm & 14x19mm I then had HCG trigger. On cd 29 had a HCG PG test with result of 82 and ultrasound showed 2 follicular cysts 23mm and 30mm.
Anyway my questions are:
1)Would femera make an already present follicular cyst bigger?
2)Could they tell at cd14 ultrasound which "follicle" was really the cyst?
3)And last according to info, which size follicle was the most likely one to have released the egg to cause pregnancy?

ANSWER: Hello Christy,

1.  Yes, an ovarian cyst can enlarged from an fertility medication.  That is why we cannot proceed if there is a cyst.  The cyst/follicle present on your second ultrasound does not qualify as an ovarian cyst, and was not the remnant of the 3.0 cm cyst that you had previously.  It was probably a normal antral follicle which can be found in the ovaries at any time in the cycle.

2.  No.  The follicles that were present were probably not related to the previous cyst.

3.  Based on the sizes of your follicles at the time of HCG trigger, you had two ovulatory sized follicles, the 30 mm and the 24 mm.  The egg could have come from either.

QUESTION: What is an antral follicle? And since it was there at the beginning of my cycle, could that follicle have matured faster and ovulated prior to my HCG trigger shot? Or is it likely that it was one of the 3 follicles present at cd14 ultrasound?

Hello Again,

An "Antral follicle" is the follicle(s) that you have in the ovary all the time, that have the eggs within.  This is an unstimulated follicle.  Usually when the ovary becomes stimulated, at the first half of the cycle, several antral follicles become activated and begin to grow.  In a natural cycle, one of these becomes the dominant follicle and ovulates, and the others then are reabsorbed.  

It is most likely one of the follicles that were present at the time of HCG that ovulated, but that is something we cannot know.
