QuestionI am a 78 year old male on medication for high blood pressure and high cholesterol. Both problems under control. I am a moderate smoker. For several months I have noticed my ankles swell up within 1 hour after getting up in the morning. Immediately upon arising the swelling from the previous day has just about completely disappeared. I would appreciate some information.

Very good question. Thank you for taking the interest in your health, and not just passing things off as "age".
It is very difficult for me to tell you exactly what is happening here. The fact that the swelling goes down at night is good. Since this is a relatively new problem, it really should be discussed with your physician. Two things come to mind most as far as worse case scenarios: congestive heart failure or peripheral vascular disease. It may be something as minor as you are just retaining some fluid. You don't mention if you have a cough or weight gain, shortness of breath at times, easily fatigued, leg pain, etc.
In any case, I highly recommended the cessation of smoking. It constricts blood vessels, makes the blood thicker and harder for the heart to pump, etc.
Please speak to your physician soon. He knows your history, medications and general health status. You may need some outpatient testing, and most certainly need an "eyes and ears" examination from your MD.
