Ankle swelling can have a number of causes. Most commonly it is from poor vein system in the legs preventing fluid from getting out of the leg. Other considerations can be a heart or kidney problems. Rarely there can be obstructions high up in the legs or pelvic organs blocking blood flow out of the legs.
