Thank you for your time and service. I hope you can give me some direction. I don't want to end up going in circles trying to figure this problem out, so I have been reluctant to go to the dr.
I am a 44yr. female. I do have multiple health problems and am feeling very defeated with all these issues. I have heart arrythmia and have a pacemaker. I fractured my spine and have alot of hardware from t12 to L3. I take coumadin, aldactone, lasix, zaroxalyn, prevacid, boniva, paxil.
Last Wed. my INR was 6, that night I started a nose bleed which the following day brought me to my ENT. They gave me alot of lidocaine and afrin in my nose and saturated afrin gauze. I began feeling very off. Completely lethargic, couldnt see straight,couldn't understand what they were saying very well or an ability to respond with more than a word or two. I was sitting in a chair reclined but i was so weak i just wanted to lay on the floor. I will add that I have nasal endoscopy regularly, so it wasnt the suction or anything that bothered me. I am very used to that. I couldn't explain what was happening but my doc. who has known my for many yrs. and through many surgeries said he never saw me look so bad. He wanted to call an ambulance, but I did say NO! I was thinking it was a reaction to the lidocaine and afrin. He was thinking I was having bleeding elsewhere in my body. Nevertheless, he called my husband and the cardiologist. I will have my INR rechecked tomorrow.
I went home and though I felt a bit shakey, was far more coherent as the day went on leading me to believe it was the lid/afrin. The following day I woke up with horrible lower abdominal pain. It is directly over my pubic bone and on the far right and left very low. It does spread across, but the intensity is in those spots. I also have horrible distention of my abdomen and it feels very uncomfortable, like i have a balloon pushing all my organs. If I eat even a couple bites my belly fills up immediately.

About 6 months ago I had an inr of 8.9. I had bruises all over and horrible lower abdominal pain which was a ruptured hemorraghic cyst. This seems to be in that location, but i didnt have the distention in my upper abdoment too. I am assuming at this point my INR has come down, as i have skipped 3 doses.
I'm so sorry. I know this is alot, I really just need someone to guide me. Should I see GYN, or Gastro. I cant have an MRI and have had too many CT scans. Please give me a couple ideas of what you are thinking it could be.

AnswerI would have a hard time blaming the lidocaine and afrin.... I would also be concerned, with those symptoms that you bled somewhere also and your pressure dropped..... and with the abd distension, bleeding in the belly or female organs.... I would get it looked at immediately.
