Understanding Low Fetal Heart Rate & Subchorionic Hematoma in Early Pregnancy

Hi! I recently went in for an ultrasound due to spotting the first two weeks of my pregnancy. On the day of the ultrasound the baby was aged at six weeks..right where we expected it. The spotting happened from week four through the first part of week five. My hcg levels were checked three times and were more than doubling. A heartbeat was detected. At the time the doctor told me 122..but when my regular doctor called back later that day they had marked 110 to 120 down for the heart rate. I was also told that I had two areas called subchorionic hemmatoma. I have no idea if I'm spelling that right! The radiologist who did the exam said that they were not always indicators of doom. The yolk sack also appeared in good shape. However when my regular doc called she said to expect a miscarriage in the next week or so. Is this heart rate too low to give a good prognosis too? Do you have any info on  uterine hemmatoma? I appreciate your help! Thank you!

The subchorionic hematoma has nothing to do with the baby's hearbeat. It is caused during the formation of the placenta when some blood gets in between some membranes of the placenta. It does not affect the baby and usually disappears on its own in a few weeks. The hearbeat should be 120-160 for a normal pregnancy. 110-120 is a bit low, but not critical. I would not predict a miscarriage based on those numbers, however, a miscarriage is always possible. YOu will have to wait and see. It is unusual to see a heartbeat before 6 weeks gestaton, and I would repeat the sonogram in another week or two.

Aloha! I really appreciate your information and help!  I had a chance to talk to my nurse again. They won't send me to a doctor, but she's being very helpful.  I think that the woman doing the ultrasound wasn't getting her information right. She offically registered the heartbeat at 120, but told my nurse over the phone it was 110 - 120. She had also told me the yolk sac looked normal. When I spoke with my nurse later the report read abnormal yold sac. I guess it is bi-lobed (what does that mean?) and slightly small and mishappen. They don't know if a twin was in there and died or if the placenta is colapsing. Hence the miscarriage diagnosis I guess. Is an abnormal sac a good indicator for that? I do have an ultrasound scheduled for next week, but am preparing myself for the worst. Thank you so much for your time!

A bilobed placenta is merely one with two lobes. It is not ominous nor does it indicate an impending miscarriage.  Your fetal heartrate is normal. As long as you are not having copious vaginal bleeding and the following sonogram shows a normally growing fetus, you do not have to worry.
