Pregnancy After 40: Risks, Concerns & Recommended Tests

Question
I just turned 42 and would like to have a second child. I was
pregnant the first time on our first try, miscarried and was
pregnant with my now 3 year old son 6 weeks later. So, we had
it very easy getting pregnant even at my advanced maternal age.
We have only been trying a few months (and not strict about it)
but I'm concerned about changes in egg production in the last
few years because of my  age. Should I go see a doctor soon?
What tests would you recommend for someone my age? Am I
being unrealistic thinking I can get pregnant at this age?

Thanks so much!

Answer
As long as you are still having a regular period, you are probably still ovulating and can certainly become pregnant. I have many patients who have become pregnant in their late forties. There is nothing you need do. You still ovulate around cycle day #14 and can conceive if you have sex on cycle day #13, 14, 15, or 16 each month. Once you become pregnant, you should be tested for neural tube defects and Down's syndrome as well as other chromosomal illness. Any woman over the age of 35 should have an amniocentesis. Good Luck.
