Understanding Missing Medication Results in Lab Tests & Medicaid Denials

I am being denied Medicade treatment. Two meds did not show up on a test, and I was accused of selling them!!!
Could you explain what factors would cause this?

1)   What were the medications?  
2)   What was the dosage of the medications?
3)   Did you take the medications?
4)   If so, when and what dosage?
5)   What lab testing was done?  i.e., a urine drug screen?
6)   If so, did you eat or drink anything before the testing was done?
7)   If so, what?
8)   If so, how long was it since you ate or drank something before the lab testing was done?
9)   What is your age, sex, and body weight?
