Understanding Recurrent Miscarriage: MTHFR & Factor XIII Heterozygosity

Question
Hello,
I'm from Romania and I need your advice. I lost two pregnancies at 7th and 10th weeks, both of them stopped from evolution. First time I got pregnant after 6 months and with the second one after 5 month of trying.
We did many tests and the karyotype is good for both of us,and the rest of the tests recommended by reproduction clinic came out normal.
We discovered that I have 2 genetic mutations MTHFR C667T and Factor XIII both heterozygous and the spermiogram of my husband is:
Volume: 2,3 ml
Total amount: 78 200 000
Consistence: normal
Motility
Progressive rapid movement: 0,0%
Progressive slow movement: 29% total amount 22 678 000
Non progressive movement: 21% total amount 16 422 000
Morphology
Normal morphology sperm proportion: 12,0%
Head defect sperm proportion: 82,0%
Round headed sperm proportion: 2,0%
Neck defect sperm proportion: 4,0%
Tail defect sperm proportion: 2,0%
We didn't repeated the sperimogram test yet. The doctor recommend us ICSI. Even I got pregnant twice they told us that the embryo is not formed from a healthy sperm and because of this the miscarriage appears.
Please tell us your opinion. Is ICSI the only chance to have a baby? Or the genetic mutation are responsible for the miscarriage?

Thank you in advance.

Answer
Hello Elisa from Romania,

You have two problems that need to be addressed, not just one.  Certainly the sperm is abnormal, but I would repeat the test before making this conclusion.  The abnormal sperm could certainly be causing the formation of an abnormal embryo that leads to a miscarriage.  However, your positive MTHFR can also contribute to a miscarriage so it will need to be treated as well.  If the repeat semen analysis is still abnormal, then you have the options of IUI (intrauterine insemination) or IVF with ICSI.

Good Luck,
